A Welcome Consultation is a no-cost telehealth conversation with a licensed nurse practitioner. It’s where we get to know you — your health, your daily life, and what matters to you — so we can point you toward the right support. It takes place by phone or secure video, from wherever you are. There’s nothing to prepare and no in-person visit.
Your consultation is a genuine look at your whole situation, not a quick form. Together we go over:
– Your current health and any conditions you’re managing
– Your daily life — getting around, food, home, who helps you
– The everyday obstacles that make it hard to follow your care
– The Medicare services and support you may be entitled to
The Welcome Consultation is a starting point, not the end. Based on what we learn together, we connect you with the right ongoing support — most often our Community Health Integration (CHI) team, whose case workers help clear practical barriers month after month: rides to appointments, food access, housing and utility issues, paperwork, and someone in your corner. When other services fit your needs — such as screenings or counseling — we guide you there too.
Yes — talking to us costs you nothing. The Welcome Consultation is open to anyone, and its purpose is simple: to understand your situation and figure out how we can genuinely help. From there, we
connect you with the right provider on our team and the support that fits your needs. Any eligibility checks happen quietly on our side — you don’t have to worry about the paperwork.
Your consultation happens remotely — by phone or secure video. A regular phone call is enough; you don’t need a smartphone, a computer, or reliable internet. A member of our team helps you schedule a time that works for you.
