● Active, unrestricted NY NP license
● Eligible to participate in federal healthcare programs (not excluded)
● Comfortable using EHR systems and telehealth platforms
● Strong documentation skills

Conduct Medicare AWVs via telehealth
● Complete HRA, medication reconciliation, screening review
● Develop personalized prevention plans
● Document thoroughly and accurately
● Identify care gaps (follow-ups coordinated by operations team)
