At MedCare we offer Medicare-covered Annual Wellness Visits through online appointments.
This visit is about preventing health issues, catching problems early and making a health
plan that’s just for you. And you can do it all from your own home.

What Is an Annual Wellness Visit?

Your Annual Wellness Visit helps make a plan to keep you healthy. This plan is based on
your health, your medical history and things that might put you at risk.

During your visit your MedCare provider will do the following:

  • Review your medical history and your family's medical history.
  • Go over the medicines you are taking. Any prescriptions you have.
  • Take some measurements, like your height, weight and blood pressure if you have that information.
  • Give you advice on how to stay healthy.
  • Talk about health tests that can help prevent problems and make a schedule for you to get those tests.
  • Discuss planning, for your care if you want to.
  • Check to see if you have any signs of memory problems.

Health Risk Assessment

As part of your visit you will fill out a Health Risk Assessment (HRA) questionnaire.


This helps your MedCare provider understand you better. They want to know:

  • Your daily lifestyle.
  • Any health problems you already have
  • Things that might put you at risk
  • Social needs that could affect your health

You can also do another questionnaire called a Social Determinants of Health Risk
Assessment. This one helps find out if social or environmental things are affecting your care.

Cognitive Screening & Mental Health Support

Your provider will perform a cognitive assessment to check for early signs of:

  • Memory concerns
  • Dementia
  • Alzheimer’s disease
  • Difficulty concentrating or decision-making

If concerns are identified, Medicare may cover a separate, more detailed cognitive
evaluation.

Opioid & Substance Use Review (If Applicable)

If you currently have an opioid prescription, your MedCare provider will:

  • Review risk factors for opioid use disorder
  • Assess your pain management plan
  • Discuss non-opioid treatment options
  • Refer you to a specialist if necessary

Your provider will also assess risk factors related to alcohol or tobacco use and recommend
treatment when appropriate.

Who Is Eligible?

You qualify for an Annual Wellness Visit if:

  • You have Medicare Part B
  • It has been more than 12 months since your Part B enrollment or your “Welcome to Medicare” visit
  • It has been at least 12 months since your last Annual Wellness Visit

You do not need to complete a “Welcome to Medicare” visit to be eligible.

How Often Can You Get It?

Medicare covers one Annual Wellness Visit every 12 months.

What Does It Cost?

At MedCare:

  • You pay $0 if your provider accepts Medicare assignment
  • The Part B deductible does not apply

However:

  • If additional tests or services are performed during the same visit that are not covered under the preventive benefit, coinsurance or deductibles may apply
  • Services like routine physical exams are not covered under this benefit
